- • Oversee medication management and coordinate residents’ medical needs with physicians, nursing leadership, resident care staff, and family members
- • Observe, document, and report significant changes in residents’ conditions
- • Recommend changes in residents’ care needs
- • Assist in supervising Resident Care Aides and ensure care is provided as instructed
- • Participate in direct and indirect nursing care, health maintenance, teaching, counseling, collaborative planning, and rehabilitation
- • Assess residents’ health status, records, and related health data
- • Analyze and interpret recorded data to determine nursing care needs
- • Plan and implement nursing interventions and incorporate prescribed medical regimens into care plans
- • Provide health education to residents and families
- • Evaluate nursing outcomes and initiate or encourage changes in care plans
- • Coordinate safe, quality care for assigned residents and families
- • Develop culturally competent, age‑specific plans of care
- • Collaborate with nurses, physicians, colleagues, residents, and families to ensure continuity of care
- • Educate residents and families about illness, treatment, and preventative measures
- • Adhere to unit policies, procedures, practice guidelines, and regulatory requirements
- • Fulfill the LPN role as teacher and responsive team member
Requirements
- Graduate of a state approved and/or accredited School of Nursing
- Minimum of 1 year of medical/surgical nursing or nursing in a skilled nursing facility required
- Current license to practice nursing from the Massachusetts Board of Registration
- BLS certification
- Staff must be vaccinated against influenza as a condition of employment
