Job summary

Qualifications

• Graduated from an accredited school of nursing.
• Associate degree or diploma required.
• Minimum 1 year recent experience in Pulmonary, Critical Care, Medical Specialty, Internal Medicine or Family Practice.
• Maintain a current Minnesota registered nurse’s licensure; WI licensure required upon hire and licensure maintained.
• Current BLS provider status in accordance with American Heart Association guidelines. Online BLS not accepted.

Responsibilities

• Assess and plan patient care using the nursing process; determine nursing diagnoses and outcomes; develop and evaluate care plans.
• Coordinate, deliver, and delegate nursing care in a timely, safe and culturally competent manner.
• Document in the EHR and reassess the plan based on patient response.
• Collaborate with the care team to ensure continuity of care and patient education using teach-back.
• Provide telehealth management and participate in quality improvements to enhance patient outcomes.
