Job summary

HealthPartners in Minnesota seeks a registered nurse to provide comprehensive patient care in a clinic setting. You will assess, plan, implement, and evaluate care using the nursing process, maintain patient confidentiality, and collaborate with a multidisciplinary team.

Responsibilities include care coordination, documentation in EPIC, teaching patients and families, and ensuring safe, evidence-based practices. Requires Minnesota RN license, BLS, and ambulatory nursing experience.

Qualifications

• Graduated from an accredited school of nursing.
• Associate degree or diploma required.
• Maintain a current Minnesota RN licensure; WI licensure on hire.
• BLS certification per AHA guidelines is required.
• Demonstrates age-appropriate patient care knowledge and critical thinking.

Responsibilities

• Assess patients and plan care using the nursing process.
• Develop and revise patient care plans with outcomes in mind.
• Coordinate, deliver, and delegate nursing care safely and ethically.
• Document care and education accurately in the EHR.
• Collaborate with the care team to ensure continuity of care.
• Provide patient and family education using teach-back methods.
