Job summary

HealthPartners in Minnesota seeks a registered nurse to provide comprehensive nursing care within a clinic setting. You will assess patients, develop and implement care plans, and document care in the EHR while upholding patient rights and privacy.

You will collaborate with the care team, educate patients and families, and participate in quality improvement activities to enhance clinical outcomes and patient experience.

Qualifications

• Graduated from an accredited school of nursing.
• Associate degree or diploma required.
• Minimum 1 year recent experience in Pulmonary, Critical Care, Medical Specialty, Internal Medicine or Family Practice.

Responsibilities

• Assess patient needs and develop care plans using nursing process.
• Plan and provide age-appropriate patient education and discharge education.
• Coordinate and document care in the electronic health record (EHR).
• Collaborate with clinicians and care teams to meet outcomes and improve care quality.
• Deliver telehealth management and participate in continuous practice improvement.
• Maintain licensure, participate in professional development and adhere to ethical standards.
• Assist with orientation and training of new colleagues when needed.
