Responsibilities

• Maintain, organize, and update electronic and paper medical records.
• Scan, upload, and accurately index medical documents into the EMR system.
• Verify that documentation is filed correctly and associated with the appropriate patient record.
• Process medical record requests in accordance with company policies, HIPAA, and privacy requirements.
• Retrieve and distribute medical records and documentation as requested.
• Review records for completeness, accuracy, and proper filing.
• Identify missing, incomplete, or incorrectly filed documentation and take appropriate action.
• Assist with filing, copying, faxing, and other administrative duties in the Medical Records Department.
• Communicate with clinical and administrative staff regarding medical record requests and documentation needs.
• Maintain patient confidentiality and handle PHI in a professional manner.
• Follow HIPAA requirements and company policies for privacy and security of patient information.
