You will assess client needs, develop individual care plans, and coordinate access to housing, healthcare, employment support, and educational programs within the Welkom community.

The ideal candidate holds a Bachelor’s degree in Social Work, SACSSP registration, and a minimum of three years of case management experience, with strong advocacy and crisis management skills.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Registered with SACSSP.
• Minimum of 3 years of experience in case management or similar social work role.
• Knowledge of social welfare policies and available resources in Welkom.

Responsibilities

• Conduct needs assessments for clients, identifying social, economic, and emotional challenges.
• Develop, implement, and monitor individualized case plans.
• Coordinate access to housing, healthcare, employment support, and education programs.
• Provide direct counselling and client support to achieve stability.
• Maintain confidential client records per agency standards.
