AHRC Nassau is seeking Direct Support Professionals in Hicksville, NY, to provide comprehensive support to individuals with intellectual and developmental disabilities. You will assist with daily activities, health, safety, socialization, and wellbeing while promoting independence and community participation.

The role requires communication skills, a valid NYS driver's license, and the ability to follow treatment plans and documentation guidelines.

Qualifikationen

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license, and maintain a qualified driver status throughout employment.
• Must have the ability to communicate and understand written and oral directives/information.

Aufgaben

• Provide ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life and independence.
• Assist individuals to become active community members and maintain natural supports and relationships.
• Help ensure a safe, clean home by assisting with household chores and participating as needed.
• Follow treatment plans and use adaptive equipment to promote health and independence.
• Assist individuals with meal preparation, including diet adherence and portion control.
• Assist with medication administration according to AMAP policy and procedures.
• Document treatment plans, medical notes, and logs to support valued outcomes and wellbeing.
• Adhere to scheduling, overtime, and staffing policies to maintain quality support.
• Attend interdisciplinary meetings to discuss progress and goals.
