Job summary

AHRC Nassau in New York is seeking Direct Support Professionals to provide ongoing guidance and support to people in the Community Living Services program to ensure a high quality of life. You will assist with self-care routines, health, safety, meals, and daily activities to promote independence.

You will work as part of an inter-disciplinary team, document plans, transport individuals to appointments, and mentor new staff.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license, and maintain a qualified driver status throughout employment.
• Ability to communicate and understand written and oral directives/information.

Responsibilities

• Provides ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life.
• Supports individuals in becoming active members of the community and maintaining natural supports and relationships.
• Ensures safe and clean homes by assisting with household chores and participating as needed.
• Follows treatment plans and uses adaptive/safety equipment to promote independence and health.
• Assists individuals with medication administration according to AMAP policy and procedures.
