AHRC Nassau is seeking Direct Support professionals to assist individuals with intellectual and other disabilities in daily activities, health, safety and community involvement on Long Island. The role emphasizes teaching life skills, promoting independence, and providing compassionate support in home and community settings.

Responsibilities include following treatment plans, documenting progress, transporting clients to appointments, and supporting staff in training and meetings.

Qualificações

• High School Diploma or equivalent preferred.
• Must have a valid NYS drivers license and maintain qualified driver status.
• Ability to communicate and understand written and oral directives/information.

Responsabilidades

• Provides ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life, promoting independence and engagement.
• Assist individuals with self-care routines, health, safety, household chores, socialization and overall wellbeing.
• Provides transportation to medical appointments, shopping, recreational outings and other needs.
• Mentors new Direct Support Staff by demonstrating supportive routines and reviewing training materials.
• Follows treatment plans and documents progress in medical case notes and logs.
• Participates in House Meetings and supports ongoing certification and training expectations.
