Job summary

The Apostle's House is seeking a full‑time Case Manager to support individuals and families experiencing homelessness. You will assess needs, develop individualized ISPs, and connect clients with housing, benefits, healthcare, and employment resources.

You will collaborate with staff and community partners to ensure holistic, client‑centered care and provide ongoing support to help clients achieve stability in housing and self‑sufficiency.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (Master’s degree preferred).
• Minimum of 2 years of experience in case management or working with homeless or at‑risk populations.
• Strong interpersonal, communication, and advocacy skills.
• Ability to manage multiple cases with varying degrees of complexity.
• Knowledge of community resources, housing programs, and social service systems.
• Problem‑solving, crisis intervention, and conflict resolution skills.
• Proficiency in Microsoft Office and case management software.

Responsibilities

• Client Intake & Assessment: Conduct intake interviews to assess housing, financial, mental health, and social service needs; develop ISPs based on assessments.
• Case Management & Advocacy: Provide ongoing support, monitor progress, adjust ISPs, and advocate for housing, employment, healthcare, and services.
• Resource Navigation: Help clients access housing, public benefits (SNAP, Medicaid, SSI/SSDI), and other services; teach budgeting and job search skills.
• Crisis Intervention & Conflict Resolution: Provide crisis intervention, de‑escalation, and collaborate with mental health professionals.
• Documentation & Reporting: Maintain client files, service plans, progress notes; ensure documentation complies with policies.
• Collaboration & Communication: Work with shelter staff, social workers, healthcare providers; attend meetings and trainings.
• Follow-Up & Aftercare: Conduct follow‑ups to ensure stable housing and employment.
