Job summary

AHRC Nassau in Syosset, NY seeks Direct Support Staff to assist individuals in the Community Living Services program, promoting independence and quality of life. You will support self-care, health, safety, meals, and daily living activities, enabling meaningful community involvement.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver’s license and maintain driver status.
• Ability to communicate and understand written and oral directives/information.

Responsibilities

• Provide ongoing support to individuals in the Community Living Services program across self-care, health, safety, household chores, socialization and overall wellbeing.
• Help individuals become active members of the community and maintain natural supports and relationships.
• Ensure a safe and clean home by assisting with household chores and participating as needed.
• Follow treatment plans and utilize adaptive and safety equipment to promote independence, health and wellbeing.
• Assist individuals with meal preparation and nutrition concepts.
• Assist with medication administration according to AMAP policy and procedures.
• Document all treatment plans and progress notes to support independence and outcomes.
• Attend meetings as part of the inter-disciplinary team and contribute information.
• Provide transportation to medical appointments, shopping, and recreational activities.
• Mentor new Direct Support Staff and review training materials.
• Participate in House Meetings and maintain certifications and training expectations.
• Perform other duties as requested by supervisor.
• This role requires fingerprint background checks and training on Personal Outcome Measures (POM).
