Job summary

AHRC Nassau is seeking compassionate Direct Support Staff to assist residents in Community Living Services on Long Island. You will support self-care, health, safety, and daily routines, while promoting independence and engagement in the community.

Training on Personal Outcome Measures (POM) and ongoing professional development are provided. Responsibilities include meal support, medication assistance, documentation, team collaboration, and safe transportation.

Qualifications

• High School Diploma or equivalent required.
• Must have a valid NYS driver's license and maintain eligibility.
• Ability to communicate and understand written and oral directives.

Responsibilities

• Provide ongoing guidance and support to individuals to ensure a high quality of life.
• Support self-care routines, health, safety, household chores, socialization, and wellbeing.
• Promote independence and active participation in daily living activities.
• Assist with meal preparation, portion control, and dietary adherence.
• Assist individuals with medication administration per policy.
• Document treatment plans and progress notes for continuity of care.
• Attend team meetings and contribute to goal progress and planning.
• Provide transportation for medical appointments, shopping, and outings as needed.
• Mentor new staff and participate in training and certification activities.
