Job summary

AHRC Nassau seeks a Direct Support Professional to provide ongoing guidance and support to people living in Community Living Services, promoting independence in daily routines and health, safety, and wellbeing.

You will assist with self-care, meal preparation, transportation, and participation in community and household activities, while following individualized plans and ensuring high-quality care and respectful interactions with residents and families.

Qualifications

• High School Diploma or equivalent preferred.
• Must submit a valid NYS driver's license, and maintain a qualified driver status throughout employment.
• Must have the ability to communicate and understand written and oral directives/information.

Responsibilities

• Provides ongoing guidance and support to individuals with intellectual and other disabilities to ensure active participation in daily life and independence.
• Provides support to become active members of the community and maintain/build natural supports and relationships.
• Ensures a safe, clean home by assisting with household chores and participating in them as needed.
• Follows treatment plans and uses safety equipment to promote independence and wellbeing.
• Supports individuals in meal preparation, including diet adherence and cooking skills.
• Assists individuals with medication administration according to agency policy and procedures.
• Implements and documents treatment plans and other agency materials to help individuals achieve valued outcomes.
• Adheres to overtime, scheduling, and staffing policies to ensure daily quality support.
• Sits as part of the interdisciplinary team attending meetings to discuss progress and goals.
