Job summary

AHRC Nassau seeks dedicated Direct Support professionals to assist individuals with intellectual and other disabilities in everyday activities, promoting independence and participation in daily life.

Responsibilities include personal care, meal preparation, medication assistance, and community outings, with emphasis on safety and respectful support. Requires valid NYS driver’s license and effective communication skills. Training is provided, with a focus on Personal Outcome Measures and teamwork.

Qualifications

• Team oriented and able to work with individuals with disabilities

Responsibilities

• Provide ongoing guidance and support to individuals to ensure active participation in daily routines.
• Support individuals to become active community members and build natural supports.
• Ensure a safe and clean home by assisting with household chores as needed.
• Follow treatment plans and use adaptive/safety equipment for health and safety.
• Assist with meal preparation including diet adherence and cooking skills.
• Administer medications according to agency policy and procedures.
• Document treatment plans and progress to promote independence and well-being.
• Attend meetings as part of the interdisciplinary team and share progress.
• Provide transportation to appointments, shopping, and outings.
• Mentor new Direct Support Staff and review training materials.
• Participate in house meetings and maintain certification and training expectations.
• Perform other duties as requested by supervisor.
