Job summary

AHRC Nassau is seeking a Direct Support Professional to assist individuals in the Community Living Services program with daily living activities and community participation. You will help with self-care routines, health, safety, and overall wellbeing, while promoting independence and engagement in daily life.

Responsibilities include meal support, transportation to appointments and outings, documenting plans, and collaborating with an interdisciplinary team. A NYS driver’s license is required.

Qualifications

• High School Diploma or equivalent preferred.
• Valid NYS driver's license required and maintained throughout employment.
• Ability to communicate and understand written and oral directives.

Responsibilities

• Provides ongoing guidance and support to individuals to ensure participation in daily living activities.
• Supports community engagement and helps maintain independence.
• Assists with self-care, health, safety, and household tasks as needed.
• Follows treatment plans and uses required equipment to promote wellbeing.
• Supports meal preparation and teaches basic cooking concepts.
• Transports individuals to medical appointments and activities.
• Mentors new staff and participates in team meetings.
• Performs other duties as assigned by supervisor.
