Job summary

Siena Francis House is seeking a Case Manager to support homeless individuals in shelter through screening, assessment, and ongoing case management. You will help clients set short term goals and connect them with wrap-around services, including housing and financial resources.

Qualifications

• Bachelor’s degree in social work, sociology, psychology or related field is required or equivalent combination of education, training and experience.
• Spanish bilingual strongly preferred.
• Excellent communication and interpersonal skills are essential.
• Two years of experience in social services and case management preferred.

Responsibilities

• Provide screening, assessment, planning, advocacy and ongoing case management for homeless shelter clients.
• Develop and monitor client goal plans; assess progress and adjust as needed.
• Maintain client records and ensure confidentiality and data accuracy.
