Job summary

You will collaborate with multidisciplinary teams, families, and external agencies to ensure safety, quality care, and timely access to resources, while upholding confidentiality and ethical service delivery.

Qualifications

• Minimum of two years in case management or residential care.
• Strong knowledge of community resources and social service systems.
• Excellent communication and interpersonal skills with diverse populations.
• Ability to maintain confidentiality and adhere to ethical standards.

Responsibilities

• Conduct thorough assessments of residents' needs to develop individualized care plans.
• Coordinate and monitor the delivery of services including healthcare, counseling, and education.
• Advocate for residents to secure resources from community agencies.
• Maintain detailed documentation of case activities and progress notes per standards.
• Provide crisis intervention and support during emergencies or heightened need.
• Facilitate communication among residents, families, care teams, and external organizations.
• Participate in regular team meetings and case reviews.
• Ensure compliance with policies, procedures, and legal requirements.
