Job summary

Qualifications

• At least one year of experience in case management or supportive services.
• Strong knowledge of local resources and social service systems in Richmond, VA.
• Excellent communication and interpersonal skills.
• Ability to manage multiple cases while maintaining confidentiality.

Responsibilities

• Conduct intake assessments to evaluate clients' needs, strengths, and challenges.
• Develop and implement individualized service plans with clients and stakeholders.
• Coordinate connections to housing, healthcare, employment services, and counseling.
• Maintain detailed and accurate case records, documenting interactions and progress.
• Advocate for clients by liaising with providers, government agencies, and organizations to remove barriers.
• Provide crisis intervention and support as needed.
• Monitor progress and adjust plans as needed; participate in multidisciplinary meetings for program development.
