Qualifications

• BS/BA degree in a human services related discipline required.
• 3–5 years' experience working with unhoused populations preferred.

Responsibilities

• Develop and monitor individualized service plans and conduct regular resident check-ins.
• Connect residents to housing, medical, mental health, and social services.
• Coordinate and track services with referrals and providers.
• Maintain accurate documentation and timely reports to supervisors.
