Responsibilities

• Assist the Physical Therapist with resident care as directed.
• Complete timely, accurate documentation in accordance with facility and payer requirements.
• Maintain a safe treatment environment following infection control and safety protocols.
• Collaborate with the PT and care team to update progress and discharge planning.
• Educate residents, families, and caregivers on treatment goals and home exercise programs.
• Facilitate ordering of necessary durable medical equipment to support independence.
