Qualifications

• Bachelor's degree in Physical Therapy required or equivalent.
• Masters/Doctorate in Physical Therapy preferred.
• Licensed as a Physical Therapist in the state; current license in good standing.

Responsibilities

• Evaluate residents to determine PT needs, functional limitations, and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT Assistants and students in resident care.
• Communicate progress and discharge plans with physicians, nursing staff, and families.
• Provide in-services and training for facility staff as needed.
• Complete timely, accurate documentation per facility and payer requirements.
