Qualifications

• Current BLS (AHA) within 30 days of hire.
• Licensed Physical Therapist in Illinois or eligible to practice.
• Bachelor’s Degree in Physical Therapy required.

Responsibilities

• Evaluate and treat patients of all ages with appropriate plans of care.
• Perform examinations and determine PT diagnoses and outcomes.
• Educate patients, families, and caregivers using effective methods.
• Develop discharge plans and adjust care based on patient response.
• Document findings in the EMR and communicate with referring providers.
• Demonstrate safe body mechanics and supervise support staff when needed.
