Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) with IDFPR license.
• BLS certification within 30 days - AHA.
• Complete required continuing education on acute stroke care.

Responsibilities

• Performs patient examinations to determine PT diagnoses and outcomes.
• Designs an appropriate plan of care that integrates goals, treatment, outcomes and discharge.
• Educates patients and families using effective teaching methods.
• Produces documentation in accordance with CTSCP67.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes.
• Communicates progress and discharge recommendations with the medical team.
