• Perform patient transfers with safe body mechanics.
• Educate patients, families, and caregivers using effective teaching methods.
• Document findings and outcomes in the EMR and CTSCP67-compliant records.
• Collaborate with referring providers and health care teams to coordinate care.
• Develop and adjust discharge plans based on patient progress and goals.
