Qualifications

Responsibilities

• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, students, and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination and selects appropriate examination methods.
• Produces documentation in accordance with CTSCP67 policy.
• Screens patients to determine need for therapy services and uses support personnel per guidelines.
• Collaborates with the medical team and provides education to patient and family.

Education

Bachelor's Degree in Physical Therapy

Licensed Physical Therapist (PT)

Basic Life Support (BLS) within 30 days
