Qualifications

• Bachelor's degree in Physical Therapy.
• Licensed Physical Therapist (PT) per IDFPR.
• Basic Life Support (BLS) within 30 days.
• Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.

Responsibilities

• Evaluates and treats patients according to plan of care and modifies as needed.
• Designs and implements treatment plans integrating goals, outcomes, and discharge plans.
• Educates patients, families, and caregivers using effective teaching methods.
• Assesses clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations using reliable methods relevant to chief complaints and history.
• Documents in accordance with CTSCP67 policy.
• Screens patients to determine need for therapy services and effectiveness of therapy.
• Supervises support personnel and provides feedback as required by regulations.
• Completes annual training for DN V hip/knee program requirements when applicable.
• Collaborates with patient and family throughout rehabilitation and provides training.
