THRIVE Wellness and Recovery, Inc. is seeking a Peer Care Manager to provide CTI services from a lived-experience perspective. This role develops person-centered care plans, connects individuals to community resources, and helps navigate health and behavioral supports.

Experience with mental illness, housing instability, or trauma is valued. You will work with a multi-disciplinary team, participate in weekly case reviews, maintain accurate EHR documentation, and be part of an after-hours on-call

Qualificações

• Must be at least 18 years old with a high school diploma or equivalent.
• At least six months of direct care experience with individuals with serious mental illness.
• Certified Peer Specialist through nypscb.org or willingness to obtain within six months.

Responsabilidades

• Provide CTI core services and care coordination for up to 15 individuals.
• Maintain documentation in the electronic health record per CTI manual and policies.
• Attend weekly case presentations and provide updates on caseload and CTI phase.
• Participate in the CTI Learning Collaborative and CTI Learning Community.
• Work as part of a multi-disciplinary team and be on after-hours on-call rotation.
