Job summary

Helio Health is seeking a Critical Time Intervention (CTI) Peer Case Manager to provide comprehensive care management for CTI program enrollees across multiple counties. This full-time role operates in a hybrid/onsite capacity and focuses on transitioning individuals from hospital to community care.

The position emphasizes peer-based support, linkage to resources, and crisis management, with a pay rate of $23.00–$24.52 per hour and a sign-on incentive for qualified licensure.

Qualifications

• High school diploma or equivalency.
• NY State Certified Peer Specialist Certification highly desired.
• 6 months of Health or Human Service experience with severe mental illness and substance use preferred.
• Case Management experience is highly desired.
• An individual does not need to be in recovery themselves but must show peer-ness via lived experiences (family or close friends).
• Must have a valid NYS driver's license.

Responsibilities

• Carry a primary caseload providing CTI services.
• Develop person-centered care plan.
• Share knowledge of and provide linkage to resources.
• Engage individuals in skill building.
• Offer crisis management.
• Collaborate with other community providers.
• Assist individuals in navigating complex systems.
• Work with service recipients' informal supports.
