Job summary

Family Service League is seeking a full-time Critical Time Intervention (CTI) Peer Specialist in Mastic, NY to provide care management from a lived-experience perspective to adults with behavioral-health needs. The role is community-based, beginning in hospital settings and continuing for about 9 months after discharge, with expert training and supervision.

Qualifications

• High school diploma or equivalent required.
• Relevant life experience and willingness to draw upon this experience to engage individuals.
• Prior experience in a behavioral health setting preferred.
• Excellent interpersonal and verbal and written communication skills required.
• Bilingual in Spanish strongly preferred.
• Valid NYS Driver's License required.
• Some evening and weekend hours may be required.

Responsibilities

• Provide Care Management from a lived-experience perspective to adults with behavioral-health needs.
• Collaborate with a multidisciplinary team including clinicians, care managers, peers, and nurses.
• Coordinate care planning and linkages with providers to ensure goal-directed care and transitions.
• Engage with collateral contacts, including family and other providers.
• Assess Social Determinants of Health and connect clients to community resources.
• Complete clinical documentation in accordance with NYS Office of Mental Health requirements in EMR.
• Provide in-person, field-based support and follow-up for approximately 9 months after hospital discharge.
