Job summary

Mentor Community Services, part of the Sevita family, seeks a Program Director for IDD Services Host Home to lead program operations, quality assurance, and stakeholder collaboration. This role emphasizes compliance, supervision, and growth within community-based supports across shifts.

The position requires a Bachelor’s in Human Services (Master’s preferred), and extensive experience with program planning, budgeting, and regulatory oversight. A valid driver's license is needed.

Qualifications

• Bachelor's degree in Human Services or related field required; Master’s degree preferred.
• Five years of related experience, with state-based exceptions (OH 1 year; PA 2 years).
• QMRP/QIDP/QDDP certification preferred.
• Current driver's license, car registration, and auto insurance.

Responsibilities

• Act as the QIDP when required; write and approve individual program plans; train and assist Program Supervisors with implementation of plans across all shifts.
• Audit records and documentation for accuracy and completeness.
• Conduct progress meetings and reviews; provide written progress reviews.
• Oversee licensing compliance and program documentation; implement compliance plan and patient rights.
• Manage funds and property; audit and monitor financial accounts.
• Implement safety strategies and timely incident reporting.
• Maintain relationships with families, case managers, contractors, and stakeholders; advocate for individuals served.
