Qualifications

• Texas PTA license active and in good standing.
• Current CPR certification.
• Professional liability insurance coverage.
• Home health or rehabilitation experience preferred.
• Ability to collaborate with a multidisciplinary care team.
• Strong clinical PTA experience and patient engagement skills.

Responsibilities

• Deliver home health physical therapy under the supervision of a licensed Physical Therapist.
• Identify patient goals and implement interventions to meet mobility needs.
• Provide evidence-based, cost-effective treatments that promote movement and independence.
• Develop individualized care plans and wellness programs.
• Educate and motivate patients to participate in rehabilitation.
• Document all services and maintain patient records.
• Collaborate with other healthcare professionals for coordinated care.
• Monitor progress and report updates to the supervising therapist.
