Job summary

Baptist Home Care, part of LHC Group and under the Optum family, seeks a Physical Therapist to assess patients in home health, develop therapy plans, and implement interventions to improve mobility and comfort. You will work with a care team to monitor progress and adjust plans as needed.

Role requires state PT licensure, valid driver’s license or reliable transportation, and CPR certification (preferred). Compensation reflects a mix of per-visit, hourly, and overall cash targets.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and needs; assists in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• T Treats patients to relieve pain and restore/maintain function.
• Instructs patients, families, and care team on plan of care and progress.
• Visits patients per Plan of Treatment; submits progress notes per policy.
• Completes evaluations and develops PT plan of care per state guidelines.
• Reports outcomes and projected frequency of care.
