Job summary

Atrinity Home Health, a part of LHC Group and a member of the Optum family, is seeking a Physical Therapist in Home Health. You will assess patient care needs related to mobility and endurance, develop a treatment plan, and perform interventions to improve well-being.

You will evaluate progress, document outcomes, and collaborate with the health care team in patients’ homes. Must hold current PT licensure, CPR, and a valid driver’s license; comprehensive benefits offered.

Qualifications

• Assess and evaluate patient care needs related to functional status and mobility.
• Communicate effectively with patients, families, and care teams.
• Document progress and outcomes per policy.

Responsibilities

• Evaluate functional status and determine therapy plan of care.
• Provide PT services within licensure scope and plan of care.
• Observe patient responses and report changes to supervising clinician.
• Treat to relieve pain, restore function, and maximize independence.
• Instruct patients and families about plan of care and progress.
• Visit patients per treatment plan and submit progress notes.
• Develop PT plan of care within state guidelines.
• Report outcomes and anticipated frequency of care.
