Job summary

Baptist Home Health, a part of LHC Group and Optum, is seeking an Occupational Therapist to assess and evaluate patient care needs in home health, focusing on functional status, activities of daily living, and adaptive equipment.

You will work with a multidisciplinary team to develop and implement therapy plans, ensuring quality patient care and adherence to CMS documentation requirements.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or access to a dependable vehicle.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy.
• Evaluates patient functional status and coordinates with physician for plan of care.
• Observes, records, and reports patient responses and changes to condition.
• Conducts patient assessments evaluating function using standard tests.
• Function in diverse home settings regardless of patient background.
