Job summary

The Pennant Group, Inc. seeks a Registered Nurse to plan, direct, and coordinate home health services in Abilene, TX. You will build on community resources to meet patient and family needs in their homes and communities.

You will supervise home health aides and LPNs/LVNs, participate in quality assessment, and handle on-call duties while collaborating with physicians and care teams to ensure comprehensive, individualized plans of care.

Qualifications

• Must be a licensed RN in the practice state
• Minimum of two years in public health or home health preferred
• Current CPR certification required
• Valid driver’s license with insured vehicle

Responsibilities

• Develops and initiates the plan of care with the patient, representative, and caregiver(s).
• Coordinates care with physicians and other health professionals as needed.
• Supervises home health aides and LPNs/LVNs (HHAs as applicable).
• Participates in on-call duties and quality assessment programs
