Job summary

Care Lync is seeking an Eligibility Specialist / Case Manager to provide case management and service coordination for adults with disabilities enrolled in the Medicaid Waiver Program. You will develop individualized care plans, broker services, and monitor outcomes to ensure cost-effective, professional care.

The role includes coordinating housing, medical, social, vocational, and other supports, with approximately 50% travel to participants as needed.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office, state-maintained databases.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and modify annually or with significant changes.
• Provide information to help participants make fully informed choices of providers.
• Ensure Behavioral Support Plan and Crisis Plan align with the ISP.
• Report incidents of abuse, neglect, or exploitation to supervisor.
• Coordinate with care coordinators to ensure seamless service delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive information.
• Attend required trainings and recertifications.
• Regularly confer with supervisors to understand participant needs and limitations.
• Travel to participants as needed (approx. 50%).
