Care Lync is a Social Services agency focusing on Support Coordination (Eligibility Specialist) to adults with disabilities. The role develops individualized care plans, brokers services, and monitors outcomes to ensure cost-effective support in home and community settings.

Responsibilities include assessing needs, creating person-centered plans, coordinating with providers, maintaining confidentiality, and traveling to participants as needed.

Formación

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• Maintain proficiency in company training and certifications.
• CPR/First Aid certification and state clearances.
• Complete and maintain required training courses.
• Valid Driver's License with personal vehicle insurance and current registration.
• Inform supervisor of changes to auto insurance or license.

Responsabilidades

• Complete assessments to identify participant needs and desires.
• Develop a Person-Centered Service Plan and update annually.
• Ensure participant can choose providers with informed decisions.
• Ensure alignment of Behavioral and Crisis Plans with ISP.
• Report all incidents of abuse, neglect, or exploitation to supervisor.
• Coordinate with other care coordinators for seamless service delivery.
• Research and develop alternative solutions for participant needs.
• Maintain confidentiality of sensitive health information.
• Attend required trainings and recertification classes.
• Regularly confer with supervisors to consider participant factors.
• Travel up to 50% to participants as needed.
