Job summary

Rise Family is hiring a Service Coordinator for its Early Intervention program in Mesa, AZ. The role focuses on supporting families of children birth to age 3 with disabilities, ensuring services align with developmental needs and family routines.

The ideal candidate has a Bachelor's degree in a related field, strong communication skills, and a willingness to travel for meetings and trainings. A bilingual English/Spanish background is a plus.

Qualifications

• Strong communication and listening skills, written and verbal.
• Willingness to travel to attend meetings, training, and appointments.
• Bilingual in English and Spanish is a plus.

Responsibilities

• Provide supports and services in the context of the family's routines and activities.
• Process referrals, schedule initial meetings with families, conduct screenings, ensure timely delivery of services.
• Maintain child files, required documentation, AZ DES system and RI SE tracking tools.
• Coordinate, attend and schedule assessments; IFSP, IEP, eligibility meetings, transition planning.
