Job summary

Rise Services, Inc. is seeking a Licensed Social Worker to support the Arizona Early Intervention Program (AzEIP). In this role, you will evaluate children's living conditions, provide counseling to families, and coordinate community resources for maximum benefit. Candidates must have a Bachelor’s Degree in Social Work, appropriate licensure in Arizona, and knowledge of AzEIP standards. This independent contractor position offers competitive pay based on experience and may require a flexible schedule for services in Phoenix and surrounding areas.

Qualifications

  • Must possess a Bachelor’s Degree in Social Work and a licensure in the state of AZ.
  • Completion of the AzEIP standards of practice for Early Interventionists within three years of start date.
  • Candidates must pass background clearance and obtain a Level One Fingerprint Clearance Card.

Responsibilities

  • Make home visits to evaluate a child's living conditions and patterns of parent–child interaction.
  • Provide individual and family-group counseling with parents and facilitate social skill-building activities.
  • Work as part of a multidisciplinary core team and participate in core team meetings.

Overview

RISE Early Intervention is a growing organization that provides support to families of children birth to three years of age with disabilities or special developmental needs, helping them to build on their child's strengths and promote growth, development and learning in their natural environment. We are seeking a Licensed Social Worker to join as an independent contractor to support our AzEIP Program and assist our EI Core Teams, which include Physical Therapists, Occupational Therapists, Speech Language Pathologists, and Developmental Specialists.

The Arizona Early Intervention Program provides support to families of children birth to three years of age with disabilities or special developmental needs, helping them learn developmental skills such as gross motor, fine motor, language, and facilitate engagement in everyday routines and activities.

Responsibilities
  • Make home visits to evaluate a child\'s living conditions and patterns of parent–child interaction
  • Prepare a social or emotional developmental assessment of the infant or toddler within the family context
  • Provide individual and family-group counseling with parents and other family members, and facilitate social skill-building activities with the infant or toddler and parents
  • Address problems in the living situation (home, community, and any center where early intervention services are provided) that affect the child\'s maximum utilization of early intervention services
  • Identify, mobilize, and coordinate community resources and services to enable the child and family to receive maximum benefit from early intervention services
  • Work as part of a comprehensive multidisciplinary core team and participate in core team meetings, one-on-one family sessions, and Individual Family Service Plan meetings
  • Be open to a flexible schedule, with willingness to work evenings or weekends as needed for service provision and to travel to attend meetings and provide services in natural settings in and around the Phoenix and/or West Valley cities
Qualifications
  • Must possess a Bachelor\'s Degree in Social Work and a licensure in the state of AZ
  • Completion of the AzEIP standards of practice for Early Interventionists within three years of a start date; candidate must demonstrate knowledge of AzEIP Policies and Procedures and the Team Based Model
  • Meet all prescribed professional requirements and rules of professional conduct as a Social Worker, including applicable licensure
  • Candidates must pass background clearance and obtain a Level One Fingerprint Clearance Card
  • Bilingual English/Spanish is a plus but not required
  • Knowledge of local communities, cultures, and resources
Additional Information

This is an independent contract position. Continuing education units are required to maintain professional credentialing for licensed, registered, or certified Social Workers. Services are supported under contract with the Arizona Early Intervention Program (AzEIP) participating agency.

RISE Early Intervention offers competitive pay depending on experience.
