Job summary

Responsibilities

  • Provide supports and services in the context of the family's routines and activities.
  • Process referrals, schedule initial meetings with families, conduct screenings, ensure timely delivery of services.
  • Maintain child files, required documentation, AZ DES system and RI SE tracking tools.
  • Coordinate, attend and schedule assessments; IFSP, IEP, eligibility meetings, transition planning.

Role Overview

RISE Early Intervention is a growing organization that provides support to families of children born to age 3 with disabilities or special developmental needs. We are hiring a Service Coordinator for our Early Intervention Program to be part of the EI Core Team.

ESSENTIAL JOB FUNCTIONS
  • Provide supports and services in the context of the family's typical routines and activities so that information is meaningful and directly relevant to supporting the child to fully participate in primary caregiving relationships and environments.
  • Process referrals, schedule initial meetings with families, conduct screenings using standardized protocol and tools approved by AzEIP, and ensure timely delivery of services.
  • Maintain child files, required documentation, the AZ DES automated system and RISE tracking tools.
  • Coordinate, attend and schedule assessments; IFSP (6 months and annual), IEP, eligibility determination meetings, transition planning; notify all interested parties of meeting dates and times.
  • Demonstrate knowledge in AzEIP policies, professionalism, child development, family professional partnerships, teaming, assessments and evaluation, planning, and intervention.
  • Demonstrate knowledge in AZ DDD/DES policies.
QUALIFICATIONS
  • Degree in Early Childhood, Child Development, Nursing, Psychology, Social Work, Special Education or a closely related field, with review of coursework and approval by AzDES/AzEIP.
  • Experience with pediatric populations (a plus).
  • Strong communication and listening skills, written and verbal.
  • Ability and willingness to travel to attend meetings, training, and appointments.
  • Must successfully complete a background clearance and obtain Level One Fingerprint Card Clearance.
  • Bilingual in English and Spanish is a plus.
