Job summary

Health New Zealand | Te Whatu Ora seeks a Needs Assessor / Service Coordinator (Disability) for a fixed-term, part-time role based in Nelson. You will coordinate disability support, work with individuals and families, and liaise with DSS providers, health practitioners, schools and community organisations.

The role is office-based with much community work, and you will join a 10‑staff Disability NASC team at the Richmond Health Hub. Eligibility to work in New Zealand or valid NZ visa is required.

Qualifications

• Experience working with people with disabilities.
• Understanding of disability models of care and support.
• Hold a full driver's licence.

Responsibilities

• Identify and coordinate support for individuals and families.
• Work with DSS providers, community services and health agencies.
• Assist NASC team to meet demand during fixed term period.
• Office-based role with community-based work.
• Collaborate with Nelson Hospital and other stakeholders.
