Job summary

Health New Zealand | Te Whatu Ora seeks a Clinical Needs Assessor– Health of Older People, based in Richmond Health Hub, to support the Needs Assessment team with service coordination and communication, working closely with Nelson Hospital and community providers.

The role requires experience with disability populations and a focus on older adults, with a permanent full-time schedule (80 hours per fortnight, 1.0 FTE), Monday to Friday, 7:30 AM–4:00 PM. Eligible to work in New Zealand is required.

Qualifications

• Experience with individuals with disabilities.
• Knowledge of disability models of care and support.
• Full, valid driver’s license.
• Experience with older adults.

Responsibilities

• Support Needs Assessment team with service coordination and communication processes.
• Build strong relationships with hospital and community providers.
• Contribute to restorative model of care and client-centred planning.
