Job summary

Health New Zealand | Te Whatu Ora seeks a Needs Assessor / Service Coordinator (Disability) for a fixed-term, part-time role based in Nelson. You will coordinate disability support, work with individuals and families, and liaise with DSS providers, health practitioners, schools and community organisations.

The role is office-based with much community work, and you will join a 10‑staff Disability NASC team at the Richmond Health Hub. Eligibility to work in New Zealand or valid NZ visa is required.

Qualifications

  • Experience working with people with disabilities.
  • Understanding of disability models of care and support.
  • Hold a full driver's licence.

Responsibilities

  • Identify and coordinate support for individuals and families.

About the Role

This role is to assist the Support Works Disability NASCteam over this fixed term period, to meet additional demands on service due tochanges to Disability Support Services and support team with operational dutiesto maintain service performance.

As a Needs Assessor / Service Coordinator in the Disabilityservices, you will work with individuals and families to identify and coordinatesupport as required. This role is office based however much of the work takesplace in the wider community.

You will be working with wide variety of people,from the individuals themselves, to providers, health practitioners, schools,and community organisations. You will work with our team based in Richmond and have closeworking relationships with DSS providers, community services, and other healthagencies, including Nelson Hospital.

We will value your commitment andexperience in working with people and their families to ensure that their needsare at the centre of their care.

Our aim is to support people whose lives are affected by adisability, to achieve their goals, retain their independence andself-reliance, and to enjoy optimum quality of live wherever they live.

This is a fixed term part-time role for 9 months,working 64 hours per fortnight (0.8 FTE), Monday to Friday 7.30AM to 5.30PM.

About you

As a Needs Assessor / Service Coordinator in the Disabilityservices, you will:

  • Be a motivated and enthusiastic individual,ideally with experience working with individuals with disabilities;
  • Have knowledge and understanding of disabilitymodels of care and support desired;
  • Hold a full driver's licence.

We are looking for a person who:

  • Has experience working with individuals with disabilities;
  • Has the ability to work in both a team environment as well as independently;
  • Is flexible and adaptable.

The successful candidate must be eligible to work in NewZealand or hold a valid NZ visa at the time of applying.
