The Department of Health and Human Services (DHHS) is looking for you to join our team and become one of our outstanding Service Coordinators within the Division of Developmental Disabilities (DDD). DDD provides funding and oversight of services to people served within the Home and Community-Based Services (HCBS) waiver program to include persons with disabilities, including physical or persons with intellectual/developmental disabilities, children with special health care needs, or the aging. A successful candidate is someone who can manage diverse relationships, is action oriented, communicates effectively and is organized.
In this position, you will serve as a case manager for people who have been determined eligible for a Waiver.
