Job summary

AHRC Nassau is hiring Direct Support Staff to provide individualized support for people with intellectual and other disabilities. The role focuses on promoting independence through self-care, health, and community participation in a supportive agency environment.

Responsibilities include guiding daily routines, assisting with meals and transportation, maintaining safety, and collaborating with an interdisciplinary team to achieve valued outcomes.

Qualifications

• Must have strong communication skills and ability to understand written and oral directives.
• Valid NYS driver’s license required and must maintain status throughout employment.

Responsibilities

• Provides ongoing guidance to individuals to ensure active participation in daily life.
• Supports individuals to become active members of the community and maintain relationships.
• Keeps homes safe and clean by assisting with and performing household chores as needed.
• Follows treatment plans and uses adaptive/safety equipment to promote health and independence.
• Assists individuals with meal planning and cooking, focusing on diet adherence and portions.
• Provides transportation to medical appointments, shopping, and recreational activities.
• Mentors new Direct Support Staff by modeling routines and reviewing training materials.
• Attends team meetings to discuss progress and outcomes.
• Performs other duties as assigned by supervisor.
