Job summary

Support Solutions in Memphis, TN seeks Direct Support Professionals to help individuals with disabilities lead more self-directed lives and stay safe and healthy in their homes and communities.

You will support daily living activities, accompany clients to appointments and activities, and document progress aligned with each ISP. The role emphasizes respect, safety, confidentiality, and collaboration with families and clinicians.

Qualifications

• Must be 18 years of age and able to read and write effectively.
• Valid driver license and clean driving record.
• Must meet background checks and safety training requirements.
• Must keep training current and complete required certifications.

Responsibilities

• Assist the person supported with personal care and daily routines per ISP.
• Provide transportation to events, job sites, medical appointments using agency or personal vehicles.
• Document daily shift notes in electronic records and track progress toward outcomes.
• Prepare meals, assist with housekeeping and laundry as needed, support independence.
• Maintain confidentiality and respect the dignity of the person supported.
