Qualifications

• Must be at least 18 years old.
• Valid driver’s license in good standing.
• Must have reliable transportation.
• Must successfully complete all background checks.
• Must successfully complete health screening (TB screening).

Responsabilités

• Assist individuals with goal completion as documented in the individualized support plan.
• Provide assistance with Activities of Daily Living (ADLs) and personal self-care.
• Safely transport individuals in company or personal vehicles.
• Accompany individuals to community integration activities.
• Complete all required documentation in the electronic health record (EHR).
• Actively engage with individuals in their home and community.
• Uphold and advocate for individual rights at all times.
• Ensure HIPAA compliance and confidentiality of information.
• Adhere to company policies and professional boundaries.
• Report any suspicion of abuse, neglect, or exploitation.
• Administer prescribed medications after training.
• Must be proficient with technology.
• Must remain awake and alert for shifts.
• Must perform physical demands of the position (lifting, transfers).
• Other duties as assigned.
