Responsibilities

• Assess people’s hopes, dreams, and desires and provide supports that facilitate achievement.
• Assist people in participating fully in their community.
• Protect and promote the rights of people receiving services and support advocacy efforts.
• Provide support to meet emotional, physical, and social needs.
• Foster person-centered, positive relationships to promote self-determination.
• Contribute to assessment and planning, and implement plans.
• Complete required documentation, including progress notes and logs.
• Assist with daily living tasks and transportation as needed.
• Ensure safety procedures and emergency knowledge are applied.
• Coordinate access to community resources and social supports.
