This full-time role involves assessment, service planning, coaching, resource coordination, and ongoing client support to promote self-sufficiency and positive outcomes. Qualifications

• Bachelor's degree in Social Work or related field; Master's preferred, minimum 3 years in case management or related human services.
• Experience in client assessments, service planning, coordinating community resources, and managing client outcomes.
• Valid MO driver's license (Class E) and auto insurance.

Responsibilities

• Conduct intake, assessment, and reassessment to identify needs and eligibility for services.
• Develop, implement, and update individualized service plans with clients.
• Coordinate services with internal and community resources; monitor progress and provide coaching.
• Maintain case records and documentation per program and grant requirements.
• Respond to client crises and liaison with community organizations to strengthen referrals.
• Perform other duties as assigned.
