Responsibilities

• Provides physical therapy services under the direction of a licensed PT and approved plan of care.
• Monitors and records patient responses to therapy and reports changes to the supervising PT and clinical staff.
• Documents supervisory activities in the patient’s medical record and participates in care conferences and quality improvement activities.
• Coordinates follow-up care and utilizes other agency resources for referrals.
